- Individuals must be either pregnant, a parent or relative caretaker of a dependent children

Vital Records of Rhode Island
The
Office of Vital Records is the authority responsible for the statewide vital records system developed to protect public health, safety and welfare. The office works for the registration and permanent filing of all vital records related to births, deaths, marriages, and civil unions for the state. The office is also the in charge of the periodic publication of statistical data derived from the records.
How to obtain a record:
One can apply for a certified copy of a birth, death, marriage, or civil union record in person or by mail. All the records of births, deaths, marriages and civil unions are with the city and town clerk offices from where records could be obtained.
- To obtain a certified copy of a record, one must present valid picture identification issued by a governmental organization which includes Driver's License, Passport, Military Identification, Certificate of Naturalization and Alien Registration Card
- The fee for a certified copy of a vital record is $20 [2].
- It takes six to eight weeks to process the request and mail certified copy to the requested person.
